When you seek treatment in the emergency room, we know you’re there for an urgent medical need. We also know that people often recover best when they can rest and relax. That’s why we offer Hospital at Home Inpatient Services to eligible patients.

This innovative program is designed to provide hospital-level care within your own private residence. Eligibility is determined by your care team in the Emergency Department based on several factors, including your condition, diagnosis, and the severity of your illness.
We deliver access, ease, and convenience right to your doorstep.
Let our medical experts, care management team, and medication come to you. If you meet the criteria for the program and choose to receive care in your home, we can handle the details. We will develop an individualized plan of care to guide you through your treatment and recovery.
This care will include:
- Daily visits with a registered nurse.
- In-person and virtual visits with a medical provider.
- 24-hour access to your care team.
- Delivery, set up, and usage instructions for medical equipment, supplies, and medication.
- Care management and post-discharge planning.
- Remote technology to enhance patient care.

If you and your doctor decide Hospital at Home is the right choice for you, your doctor or a member of the Hospital at Home team will contact you to discuss the program. Your doctor will only recommend Hospital at Home to you if it is appropriate for your condition and needs.
Hospital at Home is for patients who are sick enough to be in the hospital but stable enough to be treated at home. Some of these conditions might include:
Urinary Tract Infection
Simple Pneumonia
Cellulitis
Congestive Heart Failure
Dehydration
Chronic Obstructive Pulmonary Disease
Bronchitis & Asthma
Skin infections
COVID-19
With the Hospital at Home program, you will receive the same level of care at home that hospital inpatients receive. You will be monitored remotely 24 hours a day by a team of nurses who are coordinating and monitoring your care plan. If you ever want or need to return to the hospital, our Hospital at Home team will coordinate a safe transfer to the hospital for you.
How long you’ll be treated depends on your condition and needs. When you are ready to be discharged, your team will evaluate your discharge plan and needs and help to coordinate the appropriate follow-up services.
There is no additional cost for home hospital care other than expenses you would normally have when hospitalized. We accept all patients regardless of insurance. Health First will handle the billing for your admission and bill your insurance (as appropriate). You will receive a bill if you have a copay or receive services not covered by your insurance.
Just like in the hospital, your team will consist of Health First doctors, nurses, nurse practitioners, paramedics, and physician’s assistants.
Yes. We communicate with your inpatient doctors and value their input.
Yes. This program is an inpatient program – the only difference is the location of treatment and care. Home Care, on the other hand, is a short-term outpatient program available for anyone who has a medical condition requiring skilled care that cannot be provided solely by family members or friends.
We offer outpatient at-home services including Home Care, Hospice, and Private Duty. These services may be provided after you’re discharged if they are necessary.
