Health First Health Plans is committed to improving your health and wellness. This program is our way of
rewarding you and to say “thanks” for being loyal to your health.

How it works.

Earn rewards by completing the eligible activities below. See the reverse side of this flyer for more details.

  • Comprehensive Health Assessment (CHA).

     

  • Diabetic retinal exam.
  • Opt in for healthcare texts.


How to use rewards.

  • Your rewards balance will be loaded onto your Health First Health Plans debit card.
  • You can use your rewards balance to purchase qualifying items at participating retailers
    both in-store and online.
  • Redeem your rewards with your Health First Health Plans debit card at a variety of local stores,
    including Publix, Walmart, CVS, and Walgreens.

  • Antibiotic creams/ointments.
  • Antihistamines and allergy sprays.
  • Bandages and first aid supplies.
  • Clothes.
  • Cold/cough/flu medication.
  • Food scales.
  • Housewares.
  • Pain relievers.
  • Nicotine replacement therapy items.
  • Oral hygiene care items.
  • Pain relievers.
  • Personal care items


Online access.

Visit mybenefitscenter.com to find participating retailers, shop the online catalog, and check your balance.
 

Remember: All earned rewards must be redeemed no later than December 31.

Comprehensive Health Assessment (CHA).

1. To schedule your CHA at a Health First Medical Group (HFMG) facility or in the comfort of your home, call 321.434.6712 (TTY/TDD relay: 1.800.955.8771).
Monday through Friday, 8 a.m. to 4:30 p.m.

2. If you don’t see a HFMG provider, you can schedule an in-home CHA appointment with our vendor, Matrix, by calling 1.855.332.5286 (TTY:711), Monday through Friday, 8 a.m. to 8 p.m., or Saturdays from 8 a.m. to 5 p.m. You can also schedule online at matrixforme.com.

Healthcare texts.

Signing up for texts allows you to receive the latest updates to your plan information, along with health-related reminders to help keep you well. 

To sign up: Log in to your MyChart portal. Select your profile icon drop-down in the upper right corner. Then choose Account Settings>Notifications> Communication preferences. Turn on Text Message under Settings.

They are available Monday through Friday, 8 a.m. to 8 p.m., and Saturday, 8 a.m. to noon. From October 1 through March 31, they’re available seven days a week from 8 a.m. to 8 p.m.


*Reward balances can be used at any participating retailer to purchase any items in the store, except for these excluded items: gift cards, ATF services, alcohol, prescription drugs/services, tobacco/smoking products, CBD, firearms/hunting gear, lottery, fireworks, knives/tools, scissors/cutters, checkout bags, and tips/gratuities.
